<From entry to bathroom to shampooing, face washing and body washing>video

o The sequence of attendance on the client from entry to the bathroom to shampooing to face washing to body washing is as described below. This step of work still requires the hands of two persons - attendant and assistant - to be accomplished. Constant care must be taken that the client does not fall off the stretcher while free of safety belts.

01 The bathing stretcher is moved into the body washing position (within the reach of shower).
02 The attendant applies the foot brake to hold the bathing stretcher in position *1
03 The attendant fetches a set of towels (two bath towels and one hand towel) *2
04 The attendant puts the towels on the rack.
05 The attendant closes the door.
06 The attendant tells the client that the height of the bathing stretcher will be adjusted.
07 The attendant will adjust the height of the bathing stretcher to an easy-to-attend level by remote control.
08 The attendant and assistant undo the safety belts off the client.
09 The attendant tells the client that showering will begin
10 The attendant sprays water in the shower on the arm to check that water is at optimal temperature. *3
11 The attendant sprays water in the shower on the client’s hands and ask the client if it is comfortable. Water temperature will be adjusted to a comfortable level if necessary. *4
12 The attendant sprays water in the shower on the client’s body to warm it up and accustom it to the water temperature. *5
13 The attendant tells the client that shampooing will follow.
14 The attendant gives the client a shampoo.
15 The attendant dries the client’s hair. *6.
16.The attendant sprays water in the shower on the client’s body again to warm it up. *5
17 The attendant tells the client that face washing will be done.
18. The attendant washes the client’s face. *7
19 The attendant dries the client’s face.
20 The attendant sprays water in the shower on the client’s body once again to warm it up. *5
21 The attendant tells the client that body washing will be done.
22 The attendant wets the hand towel.
23 The attendant puts liquid bath soap to the towel and makes it lather.
24 The attendant first washes the client’s body over areas accessible without changing the client’s posture from the neck toward the feet. *8
25 The assistant draws up the client’s knees, turns the client to face the assistant and holds the client’s body in that posture.
26 The attendant proceeds to wash the client’s body over accessible areas including the back and buttocks. *8
27 The assistant restores the client’s body to the supine position with the client’s knees left drawn up.
28 The attendant hands the towel over to the assistant.
29 The attendant turns the client to face the attendant and holds the client’s body in that posture. *9
30 The assistant washes the client’s body further over areas remaining to be washed including the back and buttocks. *8
31 The attendant restores the client’s body to the supine position with the client’s knees left drawn up.
32 The assistant washes the client’s pubic region.
33 The attendant and assistant wash lather away from their own hands.
34 The attendant sprays water in the shower on the arm to check that water is at optimal temperature. *3
35 The attendant washes lather away from the client’s body over areas accessible without changing the client’s posture from the neck toward the fee.
36 The assistant turns the client to face the assistant and holds the client’s body in that posture with the client’s knees left drawn up. *9
37 The attendant washes lather from the client’s body over accessible areas including the back and buttocks.
38 The assistant turns the client back into the supine position with the client’s knees left drawn up.
39 The attendant washes remaining lather from the client’s body over areas remaining to be washed including the back and buttocks.
40 The assistant restores the client’s body to the supine position with the client’s knees left drawn up.
41 The assistant straightens out the client’s knees.
42 The attendant and assistant warm up the safety belts in the shower. *10
43 The attendant fastens the safety belts over the client.

[Note]
*1: For brake application, see [Manual for Bathing Client with Special Bathing Equipment].
*2: The assistant should stay with the bathing stretcher and closely watch the client for contingencies such as:
o Change in complexion
o Change in look
o Postural disorder
o Irregular respiration
*3: Optimize water temperature at between 38 and 40 ‘C.. Set the shower temperature regulator within this range, then spray water on the arm to make sure that it is not too hot.
*4: The client does not necessarily like a shower at optimal temperature. If the client wants to be showered at a lower or higher temperature than optimal, explain to the client for understanding why it is necessary to stick to the specified temperature range.
*5: While checking water temperature with fingers applied to the showerhead to lessen the spouting force, spray water on the client’s body starting from the farthest parts (tips of the limbs) from the heart.
*6: If the client is long haired, get a face towel ready beforehand. Using the towel, put the hair together so that it does not dip in the bath.
*7: If the client is a male or bearded, shave his face..For details, see A-f-5 [Shaving].
*8: As the pubic region is the filthiest part of the body, make it the last to wash. Therefore, leave this region intact at this stage.
*9: If the client’s drawn-up knees are stretched out, draw them up again.
*10: Warming up the safety belts is to ensure that the client does not suffer discomfort from a touch to cold belts.

<From bathing in special bathtub to departure>
o The sequence of care of the client from bathing in the special bathtub to departure is as described below. This step of work still requires the hands of two persons - attendant and assistant- to be accomplished.
01 The attendant tells the client that they will flank the special bathtub.
02 The attendant sets up the safety bar (holder). *1
03 The attendant moves to the head side of the bathing stretcher and the assistant to the feet side.
04 The attendant and assistant grips the bars of the bathing stretcher.
05 The attendant releases the brake to make the bathing stretcher free to move. *2
06 The attendant and assistant jointly position the bathing stretcher parallel with the flank of the special bathtub.
07 The attendant and assistant work together to join the bathing stretcher with the special bathtub.
08 The attendant applies the foot brake to hold the bathing stretcher in place. *2.
09 The attendant tells the client that the height of the bathing stretcher will be adjusted.
10 The attendant raises the bathing stretcher to the maximum level by remote control. *3
11 The attendant moves to flank the bathing stretcher. The assistant moves around to stand by the special bathtub.
12 The attendant tells the client that the litter of the bathing stretcher will be slid aside to the special bathtub.
13 The attendant makes the litter ready to slide by means of the lock release lever. *4
14 The attendant pushes the litter over into place in the special bathtub and makes sure that the litter is locked. *5
15 Holding the bar under the side fence of the liter, the assistant assists the attendant until the litter is locked.
16 The attendant moves to the head side of the special bathtub.
17 The assistant feels water in the special bathtub to make sure and inform the attendant that it is at optimal temperature. *6
18 The attendant tells the client that bathing in the special bathtub will be started.
19 The attendant presses the [UP] pushbutton (yellow) on the operation panel attached to the special bathtub to raise the special bathtub.
20 The assistant watches over the client and informs the attendant for immediate response in case any abnormality is observed. *7 *8
21 Upon the water surface reaching the client’s chest level, the attendant presses the [STOP] pushbutton (red) on the operation panel to stop the rising of the bathtub. *9
22 The attendant consults the clock in the bathroom for the time for which the client is in the bath so as not to let soaking in the bath exceed the designated length of time. *10
23 The assistant keeps watching over the client asking how the bath is. *7 *8
24 As the designated time for bathing elapses, the attendant tells the client that it is about time to get out of the bath.
25 The attendant presses the [DOWN] pushbutton (yellow) on the operation panel to lower the special bathtub.
26 The assistant watches over the client and informs the attendant for immediate response in case any abnormality is found with the client. *7 *8
27 The attendant makes sure that the special bathtub has lowered all the way and automatically stopped.
28 The attendant moves onto the platform at the bottom of the bathing stretcher.
29 The attendant tells the client that the litter of the bathing stretcher will be slid back.
30 The attendant makes the litter possible to slide by means of the lock release lever. *11
31 The attendant pulls the litter into place on the bathing stretcher until it is certainly locked.
32 The assistant pushes the litter out by the bar under its side fence to help it slide smoothly.
33 The attendant moves to the head side of the bathing stretcher and the assistant to the feet side.
34 The attendant tells the client that the height of the bathing stretcher will be changed.
35 The attendant adjusts the height of the bathing stretcher by remote control to an easy-to-attend level.
36 The attendant tells the client that the bathing stretcher will be moved.
37. The attendant and assistant grip the bars of the bathing stretcher.
38 The attendant releases the brake to make the bathing stretcher free to move. *2
39 The attendant pulls the release lever to make the bathing stretcher ready to separate from the special bathtub.
40 The attendant and assistant separate the bathing stretcher from the special bathtub.
41 The attendant and assistant move the bathing stretcher into the body washing position.
42 The attendant applies the brake to hold the bathing stretcher in place. *2
43 Care proceeds to the next step of work.

Bathing on Stretcher. Care in Bathroom

[Note]
*1: Have the client take hold of the safety bar that has been set up. If the client is unable to do so, still more careful attention is required to the client’s posture.
*2: For usage of the foot brake, refer to the [Manual for Care with Special Bathing Equipment] from the manufacturer.
*3: The reason why the bathing stretcher should be raised to the maximum level is to make the stretcher level with the litter mounts in the special bathtub so that the litter may be properly brought into place.
*4: Grasping the lock release lever, push the litter 10cm or so out of place by the bar under its side fence, then let go of the lever. The litter should be unlocked to become free to slide.
*5: When pushing the litter into place, mount on the platform at the bottom of the bathing stretcher with the movement of the litter. By so doing, the attendant is allowed to avoid working in an unnatural waist-stretched posture and can move the litter stably from one place to the other.
*6: Optimize the water temperature at a level between 38 and 40℃. If feeling water in the bath is not enough to determine optimal temperature, use the bath thermometer. If the temperature is too high or too low, adjust it to the optimal level in accordance with A-d-3 [Preparation of Bathroom and Dressing Room].
*7: Contingent abnormalities to check for are a change in look, a change in complexion, breathing disorder, postural disorder, fingers being caught in the machine and so forth.
*8: Typical example of action response to contingency: Rescue the client involved from the bathtub, take to the changing room and call in a nurse.
*9: Generally somewhere below the collarbone. Reasons: To prevent the client from being drown or water from entering the ears.
*10: Appropriate bathing time referred to here is 5 minutes at the longest. Consult the cloak so that bathing does not exceed this length of time.
*11: Grasping the lock release lever, pull the litter 10cm or so out of place by the bar under its side fence, then let go of the lock release lever. The litter should be unlocked to become free to slide.
*12: For specific ways of handling on-stretcher bathing equipment in use at this institution, refer to the relevant instruction manual from the manufacturer.

<From Rinsing to Departure from Bathroom>
o The sequence of attendance on the client from rinsing to departure from the bathroom is as described below. This step of work still requires the hands of two persons – attendant and assistant - to be accomplished. Constant care must be taken that the client does not fall off the stretcher while free of safety belts.
01 The attendant restores the safety bar (holder) to the original position.
02 The attendant moves to the shower side and the assistant to the opposite side.
03 The attendant and assistant unfasten the safety belts.
04 The attendant sprays in the shower on the arm to make sure that it is at the optimal temperature. *1
05 The attendant rinses out the client’s body using the shower.
06 Using the bath towel, the attendant dries accessible areas of the client’s body in the supine position.
07 The assistant draws up the client’s knees, turns the client’s body to face the assistant and holds it in that posture.
08 The attendant dries the client’s back and the litter surface in that order.
09 The attendant places another bath towel under the client’s body.
10 The attendant turns the client’s body to face the attendant and holds it in that posture. *2
11 The assistant dries other areas remaining to be dried of the client’s body and the litter surface in that order.
12 The assistant spreads out another bath towel under the client’s body.
13 The attendant and assistant turn the client’s body back to the supine position with the client’s knees left drawn up.
14 The assistant dries the safety belts with the bath towel and hands them over to the attendant. *3
15 The attendant fastens the safety belts to the client.
16 The assistant covers the client’s body with the bath towel. *4
17 The assistant opens the door.
18 The attendant moves to the head side of the bathing stretcher and the assistant to the feet side.
19 The attendant releases the brake to make the bathing stretcher free to move. *5
20 The attendant and assistant move the bathing stretcher together into the dressing room.
21 Care proceeds to the next step of work..
[Note]
*1: Optimize rinse temperature at between 40 and 42℃. To prevent the chill after a bath, the rinse temperature must be maintained within this range.
*2: If drawn-up knees are stretched out, draw them up again.
*3: Drying the safety belts is to ensure that the client does not suffer a chill from a touch to wet belts.
*4: Covering the client’s body with bath towel is to prevent the client from feeling cold due to temperature difference between bathroom and dressing room.

This is a project undertaken by Free Web College, a nonprofit organization based in Edogawa-Ku, Tokyo, Japan.
In this project, manuals and sets of sequential slide shows for care of elderly people at a nursing institution are produced and released to the public free of charge through the web site run by the Free Web College.
These manuals and slide shows are compiled in line with the prescribed theory explained in open lectures at the web site run by the Free Web College.
While Japanese versions are used in carrying on the project now, the Free Web College at this time introduces the English versions of on-stretcher bathing procedures and relevant sequential slide shows produced as a pilot (experimental trial) at its own expense.
There are other contents in the English version yet to follow, which depends on whether or not sufficient funds can be obtained from donations from those interested and translation fees from sponsors.
The project has been realized with the cooperation of [Eijuso], a social welfare corporation, and [Ouginomori], a nursing institution for the aged run by this corporation.
